Employers who were covered by San Francisco's Health Care Security Ordinance (HCSO) in any quarter in 2018 must submit the form by April 30, 2019. A failure to complete the form may result in penalties of up to $500 per quarter. Detailed instructions on completing the form can be found here. The annual reporting requirement is back after being waived for the 2019 and 2020 reporting years due to the pandemic.
